SJSM Application/Admission Requirements

A complete application to the MD program must contain the following: |
| 1. |
A completed application form. |
| 2. |
Personal essay explaining the reasons why you would like to be a physician. |
| 3. |
Official transcripts of the courses attended from school, college, or university. |
| 4. |
Two letters of recommendation on letterhead of the recommender or on the format of SJSM. |
| 5. |
Two passport size color photos (2” x 2”). |
| 6. |
A brief one-page resume. |
| 7. |
MCAT scores (optional). |
| 8. |
TOEFL Scores (if applicable). |
| 9. |
Non-refundable application fee of US $50.00 (fifty dollars). |
